4. Proof of Remarriage (Mandatory)

  • Marriage Certificate: A self-attested photocopy of the Marriage Certificate issued by the Registrar of Marriages. OR
  • Joint Affidavit (Annexure D): If a marriage certificate is unavailable, a joint affidavit signed by both you and your new spouse (as per the prescribed Annexure D format) confirming the marriage and the name change is required.

5. Proof of Name Change (If Reverting or Changing Significantly)

If you are changing your name beyond simply adopting your new spouse’s surname (e.g., reverting to an earlier name before adopting the new spouse’s name, or a more complex change), you might need:

  • Deed Poll/Sworn Affidavit (Annexure ‘E’): An affidavit in the prescribed format (Annexure ‘E’) on a non-judicial stamp paper, declaring your intention to change your name.
  • Newspaper Advertisements: In some cases, for significant name changes, you may be required to publish advertisements in two leading newspapers (one local, one national) declaring your name change.
  • Self-attested photocopy of the new spouse’s Passport: This document is listed as Optional. However, providing it helps streamline the process by offering immediate verification of your new spouse’s correct name and details for inclusion in your document.

The Step-by-Step Process

Follow the official procedure through the Passport Seva portal:

  1. Online Application: Visit the official Passport Seva website and register/log in.
  2. Select Service: Choose “Apply for Re-issue of Passport.”
  3. Specify Change: Under the reason for re-issue, select “Change in Existing Personal Particulars” and tick the boxes for “Spouse Name” and “Name.”
  4. Form Filling: Accurately fill out the online application form, ensuring your new name and your new spouse’s details are correctly entered.
  5. Payment & Appointment: Pay the required fee online and schedule an appointment at the nearest Passport Seva Kendra (PSK) or Post Office Passport Seva Kendra (POPSK).
