Documents required for ReIssue of Passport – When Passport is Stolen
These are the documents that are required for ReIssue of Passport, When the Old Passport is Stolen or Lost. Along with address, date of birth proof other documents such as affidavit and self attested photocopy of old passport should be submitted by the applicant as noted below.
