Planning a trip with your child? Even newborns require a separate passport. For minor applicants between the ages of 0 and 15, the document requirements for a Fresh Passport are specific and parent-centric.To ensure your application is processed without hassle at the Passport Seva Kendra (PSK), focus on these three universally mandatory proofs.

New Indian Passport for Minors

1. Proof of Present Address (PoA) (Document Code 7001)

Status: Mandatory (M)

Since a minor usually does not have utility bills or bank accounts in their name, their address proof is intrinsically linked to their parents. The address on the PoA must be the same as the one declared in the application.

Acceptable PoA Documents (in Parent’s Name):

  • Parent’s Passport Copy: Copy of the first and last pages of the parent’s valid passport is the most effective proof, especially if the spouse’s name is endorsed.
  • Aadhaar Card or e-Aadhaar of either parent.
  • Utility Bills: Electricity, Water, or Gas Bill (Must be recent, typically not older than 3 months).
  • Voter ID Card of either parent.
  • Registered Rent Agreement (in parent’s name).
  • Photo Passbook of a running Bank Account (in parent’s name).

Note: For minor applicants, submitting an address proof document in the name of the parent(s) is standard and accepted practice.

2. Proof of Date of Birth (DoB) (Document Code 7002)

Status: Mandatory (M)

This is the primary proof of the minor’s identity and age.

Acceptable DoB Documents (Only Certain Documents):

  • Birth Certificate: Issued by the Municipal Authority or the Registrar of Births and Deaths. This is the most essential and common document.
  • School Leaving Certificate (SLC) / Transfer Certificate (TC): Issued by the school last attended, recognized by the educational authority.
  • Aadhaar Card: If the date of birth is completely mentioned on the card.

3. Parental Declaration (Annexure D) (Document Code 8170)

Status: Mandatory (M)

This is a critical legal document providing consent and declaration from the parents for the minor’s passport application.

  • A declaration affirming the particulars furnished in the application about the minor as per Annexure D.
  • Annexure D is a declaration signed by both parents (or the legal guardian) on plain paper. It confirms the particulars of the minor and states the parent’s consent for the passport issuance.
  • Consent: It is assumed that the consent of both parents is available for the application, unless specified otherwise (e.g., divorce, separation, or single parent, which may require Annexure C instead of D).

Tip: Ideally, both parents should accompany the minor to the PSK appointment, carrying their own original passports (if available) and one set of self attested photocopies.

For children aged 0-15, the passport is typically issued with a five year validity or until the child attains 18 years of age, whichever is earlier.
